We take things for granted such as great health, ability to live a normal life and do every day activities such as going to work, travelling, spending time with the family and so on. Have you ever wondered how the life would be for those who are unable to achieve their goals because of some barriers in life such as any disability or health? Bright Stars Community Services take all those things in consideration and provide services to help the community and give them the best support so they can live a normal life. We take pride in our work and with our supportive and well-trained staff, we make sure that you get the best quality service.

Bright Stars Community Services is a Registered NDIS Provider and complies with all Victorian and Commonwealth Government requirements for the delivery of quality and safe disability support services.

Our Vision & Mission

Bright Stars Community Service’s Vision is to support Victorian participants in the National Disability Insurance Scheme, through the best service and support. The support they need to live a normal life without any barriers and achieve the goals that they want to. Our mission is to provide services that will increase educational and work opportunities for community members with a disability, including those from non-English speaking backgrounds. Solutions will be provided in a non-judgmental manner, always respecting the dignity of our clients. We are making that happen with our qualified staff and by investing in their ongoing training to ensure the best quality services to you.

Services

Assist – Life Stage, Transition
Assistance with life stages and transition, including Support Coordination. This involves strengthening a client’s abilities to coordinate their support and participate in the community.
Support coordination
Support coordination assists the individual’s ability to act independently and offers as much or as little support required. This allows individuals to control, in a sense, their own supports and encourages more community participation. Support coordination may be offered as part of an individual’s plan if there is potential they may undergo challenges navigating the NDIS or overseeing their plan effectively.
Plan Management
Provides options to engage either NDIS-registered or unregistered services to deliver required support. The administrative and financial workload is managed for the individual, which allows service continuation with trusted providers, regardless of their NDIS registration status.
Participate in the Community
This service allows individuals who have been approved for the NDIS support category, to locate and participate in activities within their community.
Assist Personal Activities
This is an individualised service (for both adults and children) where Bright Stars Community Services assist you to complete daily activities that are relevant to your individual needs and support requirements. Supports we can provide include:

• Cooking
• Shopping
• Attending medical appointments
• In home support – lawn and garden maintenance

Household Tasks
Essential daily tasks that the individual is not able to perform independently. Includes meal preparation, home and garden maintenance, cleaning and laundry.
Therapeutic Support
Tailored to assist individuals from 7 years of age to use their functional skills in order to advance their contribution and independence in day to day activities. These include language and communication skills, self-care, mobility and movement, social interactions and inclusion within the community
Community Nursing
Individuals who have high or complex needs will be appointed appropriately skilled workers to ensure ongoing specialist care.
Accommodation Support
Daily life tasks assistance and/or supervision for those in a temporary or ongoing group or shared living environment. An emphasis is placed on skills development, so the individual can live independently.
Assist – Travel/ Transport
Assisting people with special needs with travelling and transport, special arrangements are in place in order from them to get to work or for day to day travelling.
ExPhys Pers Training
Is all about health and wellness, and putting an exercise program in place that works in with your symptoms Bright Stars offers exercise physiology in a group based program or an individual assessment.
Early Childhood Supports
Early Childhood Supports, including the delivery of a variety of therapies and a dedicated worker for the family.

Children aged from 0 – 6 years old with developmental delays or disabilities may benefit from this type of support, which varies according to the child and their families’ needs.

Group and Centre-based Activities
Group and Centre-based activities, including client assistance and encouragement to participate in the community’s social and recreational activities.
Development life skills
Development of daily living and life skills focuses on training and development activities undertaken by the participant or their carer to increase their ability to live as autonomously as possible, including supports that will enhance the ability of the participant to travel and use public transport independently.
Daily tasks/Shared living
We can provide short term or ongoing support to strengthen your ability to manage your life independently.
Innov Community participation
Activities not included under other community participation registration groups.
Home modification
This service incorporates design and subsequent changes to the participant’s home. Home modification design and construction includes installation of equipment or changes to building structures, fixture or fittings to enable participants to live as independently as possible or to live safely at home.

ELIGIBILITY CRITERIA

When determining eligibility, the NDIS considers:

  • Your age
  • Your disability or significant impairments
  • Whether significant impairment is permanent or likely to be permanent
  • Whether you would benefit from early intervention.
To be eligible you must:

  • Be under 65 years of age
  • Be a Permanent resident of Australia
  • Have an impairment or condition that is likely to be permanent (lifelong) and that stops you from doing everyday things by yourself
The funded supports in an NDIS Plan will be reasonable and necessary and relate to your disability or functional impairment. These supports will help to maintain and strengthen your ability to access Bright Stars Community Services plan and will also consider how to strengthen your informal supports.

WHAT IS THE ACCESS REQUEST FORM?

To determine if you are eligible for the NDIS you will need to commence the process by calling the National Disability Insurance Agency (NDIA) on 1800 800 110 and ask for an Access Request Form. They may ask you a series of questions over the phone before sending you further documentation which will need to be completed.

WHAT YOU NEED TO KNOW

  • The Access Request Form is the first step in helping to determine if you are eligible for funded supports from the NDIA.
  • To obtain an Access Request Form you will need to call the NDIS directly on 1800 800 110 and ask for a form to be sent to you.
  • You must return the completed form to the NDIA within 28 days of the date of the letter that accompanies the Access Request Form.
  • Section 1 of the form can be completed by you or a representative for your health, for example a family member.
  • Sections 2 and 3 of the form must be completed by your treating doctor or specialist.
  • Recent reports or assessments from your GP or other health professionals that describe the extent of the functional impact of your multiple sclerosis can be included.
  • Your GP or other health professional will need to give information on six functional domains:
  1. mobility/motor skills
  2. communication
  3. social interaction
  4. learning
  5. self-care
  6. self-management
  • Once you return the form to the NDIA you should be notified within 21 working days about your eligibility and next steps.
  • Make sure you make and keep a copy of the Access Request Form and any other supporting documents.

Bright Stars can explain how the NDIS works, help you find out if you’re eligible, and guide you through the application process. If your application is approved, we can then provide you with a range of disability services under the scheme.
